In order to make sure your visit to the practice goes smoothly, we've put together all the important information about how things work at the practice.
Don't forget to bring your insurance card and picture ID with you.
In order to register and treat you promptly, we request that our patients bring their current insurance information and driver's license/photo ID with them for each visit.
We ask that on the first visit the parent or legal guardian of the patient accompany the patient.
A patient under the age of 18 must always have an adult with them at each appointment.
If you do not fill out New Patient Paperwork prior to visit we ask that all New Patients arrive 15 minutes prior to appointment time.
Please be aware that if you no show your first appointment with no prior notification it is our office policy we will not schedule another appointment for your child(ren).
Our physician believes in immunizations and it is our office policy that children are up to date on all immunizations. If you choose not immunize your child(ren) they will not be able to be patients with Gulf Coast Pediatrics.
